Our online option is the most efficient way to register for our programs. It is quick and easy to do, creates less waste, and saves your information for future programs. However, if this option does not appeal to you then print out and mail or fax in your registration using these downloadable forms. If you are registering for the Phoenix Overnight (either on its own or combined with other programs), the following packet contains all of the forms found in the standard registration packet as well as the overnight equipment list, directions to the site, and a Permission to Medicate form.
